The services include residential programs (supervised living, supported living, and independent living), community habilitation, and day habilitation. This is a permanent, full-time job. Available shift: Sun, Mon, Tues, & Wed 7am-5pm In this role you will be responsible for responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse. An ideal candidate would be a graduate of an accredited program of professional nursing, home care experience will be a plus. Required License/certification: License and current registration as a Licensed Practical Nurse in the State of New York Valid New York State Driver's License and maintain it in good standing CPR Responsibilities include, but not limited to: Provides health care to individuals receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to individuals receiving services and families as required. Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary. Interacts with a interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for assigned residences. Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews. Assists physicians in medical examinations as needed. Schedules appointments and follows up on physicians' recommendations. Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes. Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medication. *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
