FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience

Summary:

This role will be a dual role between Advanced Heart Therapies (heart transplant and mechanical circulatory support) for the Heart Transplant Program and the Independent Living Donor Advocate for the Living Donor Program. This LCSW will partner with the Social Workers and Teams to provide assessment and patient care.

Responsibilities:

  • Advanced Heart Therapies : Member of a multidisciplinary team. Responsible for the psychosocial assessment of candidates being considered for heart transplant or MCS.
  • Living Donation : Serves as advocate for potential living donors by building relationships with potential donors, assessing for coercion, assessing for ability to make informed decision, and representing the donor's best interest.
  • Participates in case reviews to evaluate care management and progress toward goals. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Periodically administers and interprets tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, implementing appropriate assessment-based treatment plan.
  • Supervises and/or trains new staff, lower level social workers, students or interns.
  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements:
  • Minimum Required Education: Master's degree in Social Work.
  • Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum Experience: None Required.
