We are seeking a dedicated Hospice Nurse Practitioner to enhance our team. Reporting directly to the Executive Director or Administrator, you will conduct Face-to-Face (F2F) encounters, gathering essential clinical information to support hospice diagnosis and prognosis. Your assessments will be shared with the hospice physician for the certification and recertification of patient eligibility.

  • Perform comprehensive physical assessments during F2F encounters in compliance with CMS regulations.
  • Deliver clinical findings, including reviewing medical treatment plans and diagnostic results, to assist the physician in recertifying hospice eligibility.
  • Make informed recommendations to the Interdisciplinary Group (IDG) based on your F2F assessment findings.
  • Communicate proactively with the hospice agency if unable to complete a F2F visit as scheduled.

Compensation for this position ranges from $58.54 to $73.17 per hour, depending on various factors including location, experience, and qualifications. At Gentiva, we believe in providing our team members with a comprehensive benefits package, which includes:

Qualifications:

  • Master's degree or higher in Nursing.
  • Completion of an accredited graduate-level nurse practitioner education program.
  • Current state license as a registered nurse.
  • Current state license or certification as an advanced practice nurse.
  • National certification as a Nurse Practitioner (ANCC or AANP).
  • Must possess a collaborative agreement.
