Social Worker

Assists physician and health care team members in understanding the significant social and emotional factors related to the health problems, participate in the development of the plan of treatment, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs and acts as a consultant to other agency personnel.
Qualifications: Bachelor's Degree in Social Work or related healthcare field required. One (1) year of hospital/health care experience preferred. Knowledgeable regarding community resources required. Excellent communication skills and ability to work in a collaborative team approach required. SC License Bachelor of Social Work (LBSW) required. Master's Degree from a school of social work accredited by the Council on Social Work Education preferred and be licensed by the S.C. State Board of Social Work Examiners as a Licensed Baccalaureate Social Worker, Licensed Master Social Worker, or Licensed Independent Social Worker required.
