Integrated MAT Opioid Treatment services are provided by an interdisciplinary team responsible for: comprehensive diagnostic assessments; participant-centered care planning; health assessments and physicals; clinical excellence and evidence-based interventions; individual, family and group therapy; psychiatric evaluations; medication management; MAT treatment (Methadone and Suboxone); linkages to additional services and care; and successful achievement of participant-determined outcomes. Responsibilities: Responsible for a caseload of approximately 60 participants. Individual therapy, family/couples therapy and group therapy. Timely, accurate, and complete documentation in Electronic Medical Record (EMR) to ensure compliance. Attends Supervision as scheduled with the Program Director Attends weekly IDT meetings. Preforms other duties as needed to ensure the success of the program. Qualifications: New York State Licensed Clinical Social Worker (LMSW, LCSW or LMHC) required. Understanding and experience with OASAS documentation requirements, including intake and assessment, medical necessity, treatment planning and clinical interventions from the start of the treatment through termination. At least 2-3 years’ experience within in the Substance use field preferably in Methadone Maintenance; other addictions, complex trauma and psychosocial stressors of poverty, abuse, abandonment, grief/loss, foster care, low/no literacy, and criminal justice involvement. Proficient in Microsoft Office (Outlook, Word, Excel & PowerPoint). Working familiarity with EMR’s. Proficient with computers and able to inspire virtual clinical care and creatively engage program participants in their care Positive attitude and demeanor with an ability to engage individuals of diverse backgrounds, perspectives, and needs. Excellent interpersonal skills and the ability to interface with regulatory agencies, personnel, community stakeholders, participants /families.
