The Hospital Liaison is responsible to coordinate all offered services for patients in a facility setting. This may include completing part or all of the admission process for patients. Partners with the admission and marketing team in developing and maintain referral source relationships, for assigned facilities. 

The ideal candidate for this position has established relationships and market connections in the Macomb County area.

Qualifications 

  • Current state license to practice as a registered nurse (RN) or licensed practical nurse (LPN).  Current CPR certification and additional certification in nursing specialty preferred but not required.

  • Demonstrated knowledge of community resources. 

  • Minimum two years experience in providing community nursing, home health care or hospice services. 

  • Prefer two years experience in marketing and community outreach.  

  • Demonstrated knowledge of payer sources and case management review. 

  • Excellent written and verbal communication skills. Excellent interpersonal skills. 

  • Must be able to travel 90% of work day and able to work nights and weekends when needed. 

  • Ability to work well with an interdisciplinary team and health care providers. 

  • Ability to establish and maintain rapport with clients and referral sources. 

  • Ability to read, analyze and interpret, professional journals, technical procedures or governmental regulations. Ability to communicate orally and through written reports and other documents relating to resident, staff and others. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public. 

  • Ability to define and solve problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical journals and deal with several abstract and concrete variables. 

Essential Functions and Responsibilities 

  • Collects patient information during the referral process. 

  • Accepts patients on behalf of the facility after clinical and financial review. 

  • Communicates with and educates referral sources regarding service via written and telephone contact 

  • Works with hospital discharge planners to facilitate a smooth discharge process. 

  • Provides assistance to hospital and assisted living discharge planners to coordinate discharge including contacting families, setting up transportation, assisting with discharge instructions, etc. 

  • Facilitates nurse to nurse communication between the facility nursing staff and hospital/assisted living nursing staff to assure continuity of care. 

  • Provide support and review the Emergency Department admissions as they present in the Emergency Department. 

  • Meets with hospital on a monthly basis to provide a review of the number of cases sent to the facility, discuss the process and modify any necessary areas 

  • Review reasons for readmissions with the hospital on a monthly basis 

  • Provides training and education to community groups as needed. 

  • Serves as a liaison between referral sources, patients and families. 

  • Prepares reports for the leadership team as requested. 

  • Admit patients to the facility service completing accurate documentation, consents, and assessments as applicable.  

  • Adheres to work schedules in completing and performing assigned tasks 

  • Applies the policies and procedures of the agency and the rules and regulations of Federal and State regulatory agencies in performing the referral and intake processes. 

  • Participates in agency quality improvement programs. 

  • Effectively assists the patient, physician and/or discharging facility to establish an effective care plan prior to the patient’s transfer to another setting. 

  • Meets or exceeds established productivity standards 

  • Recommends care alternatives which may include other community resources, when appropriate. 

  • Evaluates patients for admission to services. 

  • Advocates on behalf of patients to ensure receipt of entitled benefits. 

  • Participate and assist in the referral process to ancillary services. 

  • Adheres to work schedules in completing and performing assigned tasks. 

  • Follow up on referrals that have not been admitted to care. 

  • Partners for the development of marketing action plans for assigned facility 

  • Participates in the growth meeting and appropriate training sessions. 

  • Updates referral resources on status of patients currently receiving service or recently discharged and the patient’ response to services received 

  • Keeps current with changes and trends in the market. 

  • Demonstrates regular communication with referral sources to establish working relationships. 

  • Ensures growth and profitability of the company through the responsible use of company resources. 

  • Accepts other assignments as appropriate. 

  • Adheres to established personnel policies.
