Nurse Practitioner

HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with a Skilled Nursing Facility in Grapeview, WA.

Daily Responsibilities for Nurse Practitioner:

  • Conduct patient facing comprehensive assessments between 20-25 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions. Only 1 per day.
  • Conduct advanced illness and advanced care planning conversations for 4-5 patients/day
  • Mon - Fri 8am-5pm
Required Qualifications for Nurse Practitioner:
  • Graduate of an accredited Master of Science Nursing program
  • WA Board Certified Family, Adult, Gerontology or Adult-gerontology Acute Care NP
  • 1+ years of clinical experience in NP Roles
  • Effectively manage acute and chronic medical and behavioral conditions

Preferred Qualifications for Nurse Practitioner:
  • Doctor of Nursing Practice
  • Meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Advanced Illness and end of life discussions

Pay Details: $80 - $95 an hour
