Responsible for the comprehensive delivery of medical nutrition therapy using evidence-based practice for obesity management. Screen, assess, and implement nutrition care plans.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Obtains comprehensive data on individual patient's personal, medical/health histories including anthropometric, lab results, medications, demographics and activity/exercise.
- Obtains detailed nutrition history utilizing appropriate interviewing techniques.
- Determines nutrient and energy requirements.
- Analyzes and interprets nutrition and health assessment data, identifies nutrition related problems and serves as coach in helping patients set and meet realistic and measurable personal goals.
- Utilizes appropriate counseling techniques to work with patients to formulate nutrition care plans.
- Develops individualized food plans for patients based on their level of glycemic control, medication and exercise regimens.
- Establishes appropriate follow-up plans and continues counseling patients to obtain appropriate clinical outcomes.
- Documents nutrition and health assessments using program electronic medical record templates at all times.
- Meets productivity standards in counseling and documentation.
- Obtains required continuing education credits yearly and maintains documentation of them.
Qualifications

Demonstrates ability to communicate and relate to a diverse group of patient, colleagues, and other health care professionals.
Demonstrates nutrition education and counseling skills to patients/clients to achieve desired goals.
Demonstrates ability to prioritize work with limited resources.
Demonstrates ability to effectively make decisions that achieve optimal outcomes for patients.
LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable)
Registered Dietitian - Commission on Dietetic Registration - required
Licensed Dietitian - Commonwealth of MA - required
EDUCATION
BS in Nutrition, Dietetics, or related subject required / MS preferred
EXPERIENCE
Clinical experience desirable but not required
