The RN Case Manager is responsible for the overall direction of home health clinical services for skilled and non-skilled care. The RN Case Manager will have overall responsibility for the quality, adequacy of services provided and supervision of staff with the opportunity for advancement to a full time position as Assistant Director of Nursing. The RN Case Manager establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality.

Responsibilities of RN Case Manager:

  • Work collaboratively with the Director of Nursing and the entire care team to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits.
  • Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs, along side the Director of Nursing.
  • Manage emergency situations in a prompt, precise, and professional manner.
  • Provide nursing care in accordance with applicable regulations and Company policy.
  • Assisting agency director with all agency functions, and performance improvement projects
  • Orienting and training new nurses, and supervising and educating nursing assistants
  • Reviewing and updating care plans, clinical records, documents, as well as providing clinical supervision in compliance with the company and government standards, alongside the DON.
  • Occasional on-call responsibilities required.
  • Tracheostomy care and experience, ostomy care and experience, medication management, venous puncture, wound management, catheter care
  • IV infusion experience including the ability to perform all types of IV administration and insertion of a peripheral IV

  • Ability to function as a team player as well as independently when needed
  • Ability to be a leader and manage time appropriately
  • Ability to handle situations with tact and ease
  • Excellent independent assessment skills
  • Previous experience with home health care
  • Good customer service experience

Requirements/Qualifications for RN Case Manager

  • Valid RN license in the state of Pennsylvania
  • Bachelor's degree in nursing (BSN) preferred
  • Minimum three (3) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or similar health facility preferred.
  • Reliable car/auto insurance
  • Clean background and criminal record
  • Understands regulatory compliance with state and federal regulations.
  • Adhere to HIPAA and maintain client confidentiality.
  • Possess exceptional nursing assessment skills.
  • Comply with accepted professional standards of practice.
  • Proven ability to build quality and implement improvements within the operation.
  • Analytic/Critical thinker.
  • Policy development and case management experience is a plus.
  • Proven leadership skills with the ability to effectively supervise and direct staff.
  • Strong knowledge of medical procedures and protocols.
  • Excellent communication and interpersonal skills.
  • Ability to multitask and prioritize responsibilities.
