Pay Range: Starts at $45,359/year. Based on your education, experience, and credentials.

Schedule: (Full-Time - Hybrid)
  • Monday - Friday: 8:30 a.m. - 5:00 p.m.
What You Will be Doing

Primary job responsibilities involve receiving and coordinating all referrals coming into the CME, managing reporting, and recording transitions of levels of care within the CME and exits from the CME. The Program Coordinators will conduct initial outreach to explain the service, obtain consent for participation, and link the individual or family/ caregiver to the appropriate care coordinator. Additionally, the Program Coordinators will serve as a liaison and support for individuals and families/ caregivers requesting enrollment in the 1915(c) to access available services, including submission of prior authorizations and assisting with the processes related to accessing flexible funds.

Minimum Education/Experience:
  • Bachelor's degree in psychology, social work, or other related field OR at least three years' relevant experience with the ability to complete additional training, as necessary
  • Three years of experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers
  • High School and GED
  • Experience in one or more of the following areas of expertise:
    • Family systems
    • Community systems and resources
    • Case management
    • Child and family counseling or therapy
    • Child protection
    • Child development
Additional Eligibility Qualifications:
  • Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record
  • Valid car insurance
  • Current CANS Assessor training or the ability to complete such within 90 days of hire
  • Training in cultural competency or the ability to complete such within 90 days of hire
