General Purpose

Under the supervision of the Assistant Director of Health Services, and indirect supervision of the Director of Nursing & Health

Services, the LPN directs and manages resident care including resident assessments, administration of medications, care, planning, supervision of staff, licensure compliance, and being a liaison for residents, family and staff.

Customer Service: Responsible for effectively meeting customer needs by caring out established customer service / guest relations protocols of SGV. Meeting the goal of having a very high degree of responsive and friendly customer service for all customers and taking responsibility for customer satisfaction.

Essential Duties

  • Managing and supervising Certified Nursing Assistants regarding all areas of resident care. Assist with health-related resident concerns as needed
  • Administration of medications as ordered by physician and documents according to policy.
  • Monitors residents for changes in condition and notifies appropriate personnel; communicates observations to care partners and unit manager.
  • Collaborates effectively with other healthcare providers (therapy, pharmacy, hospice, lab and private caregivers, etc.)
  • Implements resident care plans and modify changes plans of care according to changes in residents' condition .
  • Completes new resident nursing assessment for move-ins to the neighborhood and initiates the nursing & CNA care plan s.
  • Communicates relevant information from new resident assessments to care partners
  • Regulatory compliance in the TOL Neighborhood:
  • Supervises shift rounds by CNA's
  • Monitors &/or performs assigned tasks for shift
  • Oversight of resident assignments of the CNA's
  • Has knowledge of nutrition and hydration requirements of resident population
  • Facilitate pharmacy communication for medication refills, medication changes, and stat orders.
  • Conci1Jct audits of the medication administrates record (M.AR.} as assigned.

• Responsible for providing accurate information in resident's record, according to SGV policy and in accordance with

regulatory guidelines.

o Completes required weekly, monthly, and quarterly nursing assessments as ordered and assigned

o Proactively identifies improvement opportunities and establishes action plan

• Collaborates with Nurse Manager on:

New care partner orientation

6 C.N.A.Care partner performance evaluations

o Effective progressive correction action

• Responsible for delegating staffing to ensure proper resident care.

o Assist with scheduling for resource allocation

• Responsible for serving as a liaison between physicians, family, signif

icant others or responsible parties as it relates to

resident care (as needed).

o Provides relevant resident data to supervisor for quarterly care planning meetings.

• Responsible for supervising/monitoring meals service in the dining rooms and collaborates with dietician in identifying at residents.

• Complies with established policies and procedures.

• Participates ln committees as assigned.

• Responsible for maintaining a safe work environment

e Responsible for notifying Supervisor of safety hazards

o Responsible for identifying equipment malfunctions and reporting to the appropriate personnel

• Mentors and trains new care partners as needed.

• Attends education courses and trainings as assigned/scheduled

• Other duties as assigned.

Qualifications

Level 1: One year certificate from college or technical school; and

two years related experience and/or training; or equivalent

combination of education and experience.

Level 3: Ability to read, analyze and interpret documents such as

safety rules, operating and maintenance instructions and

procedural manuals. Ability to write reports and correspondence.

Ability to speak effectively before groups of customers or

employees of the organization.

Level 2 : Ability to add, subtract, multiply, and divide in all units of

measure, using whole numbers, common fractions and decimals.

Ability to compute rate, ratio and percent and to draw and interpret

bar graphs.

Level4: Ability to solve practical problems and deal with a variety of

concrete variables in situations where only limited standardization

exists. Ability to interpret a variety of instructions furnished in

written, oral, diagram, or schedule form.

Current Georgia LPN License with at least two years of related

experience.

Possess good communication and inter-personal skills, documentation, and assessment. The ability to read, write and follow written and oral instructions in English. Current CPR training required.

The work week can be up to 40 plus hours per week on a year-round basis: however, the position requires a willingness and flexibility in work hours when necessary to fulfill job responsibilities, including occasional evening or weekend work. Employee is subject to personnel policies and procedures prescribed for all personnel.
