Compensation: $16.80 - $21.00 per hour (based on years of experience)

Summary:

The Care Navigator plays a pivotal role within the CareCoordination Division of the Quality Department. Reporting directly to the CareCoordination Manager, the Care Navigator is an integral member of collaborativepods alongside Care Coordinators. This position is focused on enhancing patientcare journeys by providing support, guidance, and coordination to ensureseamless transitions through various healthcare services.

Principal Responsibilities:

1. Willparticipate and maintain a culture within The Corvallis Clinic that isconsistent with the content outlined in the Service and Behavioral Standards document. To this end, employee will be expected toread, have familiarity, and embrace the principles contained within.

2. Assists CareCoordinators to schedule appointments, diagnostic tests, and consultations asrequired. This can include assistance in arranging transportation or logisticalsupport for patients who require assistance.

3. Performstuck in, check in and follow up calls to high-risk patients, providing supportto patients with education on self-care techniques, disease managementstrategies, and methods for actively participating in their treatment plans asdirected by Care Coordinator(s).

4. Updatesresource lists for patients; identifying relevant community resources, supportprograms, and services that align with patients' needs and preferences, and assistspatients in accessing these resources to bolster their overall well-being andfacilitate recovery.

5. Engages withpatients to understand their medical history, treatment plans, and care goalsand works in tandem with assigned Care Coordinator(s) to develop relevant plansand approaches to care.

6. Ensuresaccurate and timely documentation of patient interactions, interventions, andoutcomes.

7. Providesemotional support to patients and their support person(s), addressing theirconcerns and addressing any uncertainties they may have.

8. Facilitatescommunication between patients and healthcare providers, ensuring that allparties are well-informed about care plans and progress.

9. Serves as avocal advocate for patients' rights, preferences, and needs within thehealthcare system, and escalate concerns or challenges faced by patients andtheir support persons to appropriate parties.

10. Participatesin team meetings and contributes to the collective goal of optimizing patientcare.

11. Collaborateswithin the pod team to identify potential challenges or barriers to effectivepatient care and contributes to innovative solutions and suggestions to improveprocesses and patient experiences.

12. Occasionally works at other Corvallis ClinicSites as assigned.

Education/Licensure/Experience:

1. High school diploma or equivalent required.

2. One of the following is required:

- National certification as Medical Assistant,LPN, or CNA II.

- Advanced coursework in a healthcare-relatedfield.

- Minimum two (2) years of experience working incommunity or health care setting.

3. Experiencein the following preferred:

- Outpatient,ambulatory care and/or community care

- Carecoordination

- Electronicmedical record (EMR) system

Knowledge and Skills:

1. Knowledgeof healthcare systems, medical terminology, and patient care processes.

2. Demonstratedinitiative, ability to work with others in an engaging manner, and goodprofessional judgment.

3. Proficiencyin using relevant software and tools for documentation and communication.

4. Empatheticand patient-centered approach to care navigation and demonstrated commitment toimpacting the care of high-risk patients.

5. Excellentorganizational skills and attention to detail for accurate documentation andcoordination.

6. Problem-solvingability and adaptability in a dynamic healthcare environment.

7. Strongcommunication skills to effectively interact with patients, colleagues, andhealthcare providers.
