Job Description
Job Title: Physical Therapist - Home Health

Job Specialty: Physical Therapist

Job Duration: 13 weeks

Shift: Days, Monday-Friday; one weekend a month rotation

Guaranteed Hours: 40 hours per week

Experience: Minimum of one year of Home Health experience

License: CA PT License

Certifications: N/A

Must-Have:
- CA PT License
- One year of Home Health experience

Job Description:

- Prioritize start of care visits and supervise Physical Therapy Assistants
- Mandatory next-day reporting of start of care visits
- Ensure clinical notes and other documentation are completed within 48 hours of visit
- Maintain timely communication with supervisor, intake, and office
- Be generally responsive during normal business hours
- Territory includes Alameda County, with potential expansion as needed
- Training includes 4 full days, conducted in-house
- Documentation system: Kinnser
- Equipment issued includes iPad and Ring Central
- Mileage reimbursement available per mile from the first to the last patient
- Company car option available with adjusted terms
- Productivity requirement: 30 points per week
- Point to hourly conversion: 1 point equals 1.33 hours
- Average workday consists of a caseload and visit mix equating to 8 hours per day and 40 hours per week
- Additional work time must be pre-approved by the Clinical Supervisor
