Position Description

Summary of Responsibilities:

Under the general direction of the MH Regional Director, this person is responsible for the oversight and operation of the Blended Case Management Unit for adults and children. This person will provide supervision and support to ensure that individual needs are met. By supporting and ensuring that needed services are provided and quality standards are met, this person will ensure compliance standards are achieved. Duties are provided in accordance with all existing policies, procedures, and regulations and in accord with the mission and values of the company.

Essential Functions:

Supervision, Coaching, & Training.

Support, coach, and train (as needed) assigned case management staff. Ensure that case managers complete required work and that assignments are completed and achieve desired standards for quality, compliance, and service delivery. As needed, recommend training to enhance skill and professional development.

Oversight & Management.

Ensure assigned work is complete and meets desired standards for compliance and quality. Ensure service delivery standards are known and support staff (as needed) in achieving expected standards. Review assigned work and as needed make recommendations for improvement. Ensure that current policies and procedures are adhered to and as needed educate staff regarding relevant policies and procedures. Collaborate with other team members to identify trends and offer recommendations for improvement.

Collaboration & Communication.

Partner with assigned team members and colleagues to ensure that program objectives and outcomes are achieved. As part of the collaborative process, facilitate and attend department and team meetings, training sessions, other meetings as deemed necessary.

Tracking & Reporting.

Ensure that required local, state, and federal reporting requirements are completed including ensuring that any program/department/unit-specific evaluations or reporting requirements are completed. Reporting and tracking requirements may vary during the year and by location.

Coverage & Availability.

Ensure adequate coverage within the unit. When needed, assist, and serve as a case manager if necessary. After-hours work may be necessary.

Education & Experience:

Master's Degree in Sociology, Social Work, Psychology, or related field. Bachelor's Degree in Sociology, Social Work, Psychology, or related field and five (5) years' experience, two (2) of which must be in supervisory roles. Bachelor's Degree in nursing and three (3) years' mental health experience.

Employment Requirements:

Automobile and Valid Insurance, Valid Pennsylvania Driver's License and Acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance

Working Conditions:

Predominately in-office work within an open environment. Travel throughout the assigned region may occur and may be in adverse weather conditions. Work may include periods of on-call during the evening and weekend hours. Must be able to lift up to 15 pounds.
