Overview

HarmonyCares is seeking a Hospice RN Case Manager to provide high-quality, coordinated health care in patients' homes. The role includes intermittent skilled nursing services and supervising hospice aide staff. Join our team and help bring personalized, quality-based healthcare to those in need.

Responsibilities:

  • Admit eligible patients to hospice services
  • Assess patient needs and establish care plans
  • Communicate patient status to clinical leadership and attending physician
  • Ensure use of the 4Ms for Age-Friendly Care

Qualifications:

  • Current unencumbered RN license in the state of practice
  • Valid driver's license and good driving record
  • Ability to make sound nursing judgments
  • Formulate individualized patient care plans

Pay Transparency:

Compensation based on various factors unique to each candidate.
