JOB SUMMARY:
The Licensed Clinical Social Worker (LCSW) I provides inpatient and outpatient comprehensive clinical social work practice to all patients with psychosocial and psychiatric needs. The Licensed Clinical Social Worker (LCSW) I provides psychotherapeutic intervention, psychosocial assessment, crisis intervention, evidence-based practice, group and individual therapy, and other clinical interventions and treatments as per physician order or per protocol. The Licensed Clinical Social Worker (LCSW) I also provides discharge planning and utilization review to ensure appropriate benefits and best clinical and discharge outcomes. In collaboration with members of the inter-disciplinary healthcare team, the Licensed Clinical Social Worker (LCSW) I contributes to the development and implementation of the multidisciplinary plan of care for patients; provides input in determining the appropriate patient status and level of care; and functions as a key linkage between the physician, staff, and hospital leadership in the day-to-day management of appropriate and efficient patient care.
JOB SPECIFICATIONS:
A. EDUCATION:
Master's degree from an accredited school of social work or social welfare required.
B. EXPERIENCE:
Prior work experience in a health care environment is required. Acute care hospital experience preferred.
C. PREREQUISITE SKILLS and KNOWLEDGE:
1. Knowledge of psychosocial aspects of illness including family systems theory, crisis theory and effective treatment modalities used to assist patients and their families in adjusting to the debilitating effects of illness.
2. Knowledge of age specific theories of human development; social work theory and practice as well as applicable community resources and legal aspects of care for varying populations and assigned hospital units.
3. Significant independent judgment, analytical, problem-solving and collaborative skills in order to organize and prioritize multiple tasks, providing patient centered leadership and direction.
4. Adhere to professional ethics, practice, and values as delineated by the National Association of Social Workers (NASW) Code of Ethics.
5. Demonstrates knowledge, humility, ability and willingness to engage in continuous learning about various ethnic, spiritual, and culturally diverse backgrounds.
6. Demonstrates expertise and knowledge of DSM V.
7. Demonstrates expertise and knowledge on reporting laws as well as all legal aspects of care (W & I Codes, Title 22, CMS, EMTALA, DOJ, Probate Codes, etc.) relevant to specific population serviced. Specific knowledge of child, elder, and dependent adult mandated reporting law required.
8. General knowledge of available health care and community resources appropriate for populations served is required; in-depth knowledge is preferred.
9. Demonstrates compliance in continuous learning and professional leadership skills.
10. Demonstrates knowledge of government and private insurance benefits (e.g. Medi-Cal, Medicare) and the managed care system and utilization review procedures.
11. Must have the ability to read, write, and follow English verbal and written instructions, and have excellent oral and written communication, interpersonal, problem-solving, conflict resolution, and time management skills.
12. Proficient computer skills are required including the use of Electronic Health Record (EHR) and other IT applications and platforms.
13. Working knowledge of Inter-Qual criteria.
LICENSE / REGISTRATION / CERTIFICATION:
Current Licensed Clinical Social Worker (LCSW) with the California Board of Behavioral Sciences (BBS).
Certified as a Basic Life Support provider (BLS-HCP) within 90 days of hire.
Integrative Agitation Management (IAM) and TEAM Advanced® Certification upon hire, and maintained annually.
DUTIES AND RESPONSIBILITIES:
D. ESSENTIAL (not modifiable)
1. Clinical
a. Provides psychosocial assessments and treatment to patients and or families related to adjustment to illness and discharge planning.
b. Demonstrates knowledge and skills necessary to provide cultural, spiritual and age specific care by obtaining specific care information and assessing relevant information needed to identify each patient's unique treatment and discharge planning needs.
c. Performs unit/department/program specific comprehensive psychosocial assessments, ensuring pediatric, adult and elderly patient's age related needs and coping mechanisms are clearly identified.
d. Acts as patient advocate and resource regarding patient's needs including financial considerations.
e. Possesses clinical expertise to effectively assess, coordinate, implement and evaluate all services required to meet the needs of the patient.
f. Provides individual, conjoint family and group therapy as appropriate to setting.
2. Collaborative
a. Maintains effective communications with staff and attending physicians related to patient's psychosocial and psychiatric needs.
b. Maintains open communication and positive working relationships with all hospital departments as well as adheres to hospital chain of command.
c. Willing to assist others and supports other hospital personnel in providing optimal patient care.
d. Demonstrates a clear understanding and adherence to designated unit/department/program as well as overall policy and procedure.
e. Collaborates with physicians, patients, families and treatment team members in the development of the patient's plan of care.
f. Assist and promotes patient/family education and ensures that the patient's educational needs are being met.
g. Works with public and private sectors (i.e. public guardian's office) to ensure best treatment outcomes as well as completes necessary documents consistent with Licensed Clinical Social Worker scope of practice.
h. Ensures proper content, application, and submission of required legal documents impacting patient care and treatment outcomes (i.e. LPS, Probate, mandated reporting).
i. Provides clinical information for placement and referral to outside agencies consistent with HIPAA and state guidelines for special needs populations.
j. Interprets and cites applicable laws and regulations to staff and physicians pertinent to individual patient needs.
k. Provides psychosocial education and perspective to other healthcare professionals, including nursing students or any other healthcare related practice, as indicated.
l. May provide clinical supervision to master's level associate clinical social workers or graduate level MSW interns.
m. Participates in meetings and committees relevant to specific treatment area/department/program and represents MarinHealth Medical Center at relevant community meetings and committees.
3. Discharge Planning
a. Maintains and provides current information and referral services to patients, caretakers and families related to appropriate community resources and agencies.
b. Independently case finds, coordinates and implements discharge plans for all patients with psychosocial needs.
c. Maintains knowledge of current eligibility criteria for a wide array of community resources.
d. Maintains positive working relationships with community agencies.
e. Utilizes and expands knowledge base of community resources.
f. Initiates contact with state, county and private resources, including family, to facilitate discharge to the least restrictive level of care.
g. Provides advocacy for clients in accessing appropriate community based resources.
4. Documentation
a. Provides accurate, clear, concise, relevant and timely documentation in patient's charts accordingly to individual units/departments/programs documentation system and their adherence to specific CMS and State regulations and in accordance with policy and procedure.
b. Maintains consistent and clear documentation on daily assessment of patients related to care plan.
c. Documents timely initial psychosocial assessment according to unit/department/program's standard of care.
d. Documents all collateral contracts.
e. Documents all screening and clinical interventions (i.e. SBIRT, AUDIT, DAST, CAGE, C-SSRS, ITSS).
f. Documents all discharge planning efforts, including timely insurance reviews.
g. Documents treatment planning according to each units/departments/program standards by using each unit/departments/programs specific forms.
h. Maintains confidentiality of privileged communication with patients and families adhering to Clinical Social Work Practice.
