Conduct annual recertification of households and units to determine eligibility Conduct monthly contacts of each household and document in electronic medical reporting system. Conduct assessments and develop service plans to assist households reach optimal independence Demonstrate developing knowledge of HIV/AIDS housing. Customer service skills with the ability to work with population experiencing past and present traumas. Coordinate grant funded and unfunded homeless prevention services Responsible for completing emergency assistance applications that assist HIV+ and non-HIV+ households from becoming homeless. Participate in MCPEtE (Monroe County Partnering to End the Epidemic) and HSN (Housing Services Network) monthly meetings and other housing related activities in the community. Miscellaneous Requires the ability and commitment to respect and support inclusiveness and diversity including but not limited to individuals of different backgrounds, cultures, races, ages, sexual orientations, gender identities or expressions, experiences, opinions, etc. Requires individual demonstration of commitment to the One Trillium behaviors and business impacts and modeling them in the organization. Responsible for maintaining confidentiality of all patients, clients, employee, protected and proprietary information. Demonstrate ability to collaborate effectively in a team setting. Ability to meet with program participants in their home. Computer skills required including but not limited to the ability to work within a Windows OS, and the ability to create, save, open, close, and forward electronic documents and emails. Ability to organize your day and work independently as well as be flexible and work in a team environment. Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans. Other duties as assigned. Qualifications Associate degree in human services field preferred, high school diploma or equivalent required. Ability to speak Spanish and experience working with diverse populations preferred. Minimum two years of experience in residential services or advocacy, plus knowledge of affordable housing, subsidy programs, valid NYS driver license and reliable transportation required. Physical Requirements While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk, and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
