General Description:

The Well Woman HealthCheck Program helps low-income, uninsured, and underinsured women gain access to breast and cervical cancer screening and diagnostic services.

We are looking for a Patient Navigator that has an interest in guiding patients through the healthcare system to ensure timely screenings, diagnosis, and treatment. The Patient Navigator must be computer savvy and enjoy working in an office environment completing and processing large amounts of paperwork and computer data entry with accuracy and within specific timeframes.

Essential Duties:

  • Contact clients for annual follow-up by phone and by mail
  • Intake and verify program eligibility of new and returning clients
  • Route ineligible clients to other services such as the Market Place, Medicare or sliding fee services
  • Identify services for which each client is due and schedule appropriate medical appointments
  • Assist with cancer screening and diagnostic education of clients and clinic patients, as needed
  • Track clients, appointments and screening & diagnostic outcomes in appropriate spreadsheet and/or database and follow-up with clients as needed
  • Secure schedule template and assist with planning and implementation of screening events
  • Help interpret for our clients as needed
  • Communicate well with team and medical providers to ensure high quality care for our clients and proper utilization of other grant funding

Skills/Position Requirements

  • Excellent customer service skills
  • Must keep private health information confidential and comply with HIPPA guidelines
  • Flexibility to work an occasional weekend day or travel out of town for screening days
  • Competency with Microsoft Office, specifically Word, Excel and Outlook
  • Ability to accurately enter and pull data from our FileMaker Pro database
  • Excellent team skills including communication, support and strong work ethic
  • Professional verbal and written communication skills a must
  • Associates Degree or equivalent related experience

Bilingual Spanish/English Speaking is preferred for this position
