Assistant Medical Director for Primary Care - Breaking Ground

The Assistant Medical Director for Primary Care - Breaking Ground, in partnership with the Director of Primary Care, Chief Medical Officer, and other agency leadership, is responsible for assisting in the development, implementation, administration and leadership of Janian's primary care operations in Breaking Ground programs. This leadership includes supervision of other Janian staff, and administrative responsibilities at Breaking Ground. The Assistant Medical Director for Primary Care - Breaking Ground is a Janian Primary Care Provider whose service time includes administrative time in lieu of clinical service hours to fulfill this role. The Assistant Medical Director for Primary Care - Breaking Ground reports to the Medical Director for Psychiatry - Breaking Ground.

Clinical Responsibilities - 60% of position
  • Clinical responsibilities will fill 3 days of a 5-day work week.
  • Provide medical care to agency clients. This includes but is not limited to:
    • Gathering patient clinical histories; this includes but is not limited to:
      • Speaking directly to patients in person and/or remotely.
      • Gathering collateral history from collocated staff, family and/or friends.
      • Conducting chart reviews both internally and externally.
    • Conducting general, in-depth, and focused medical examinations as clinically indicated, in whatever location the visit is occurring (such as home, bedside, office, clinic, street, etc.).
      • This includes conducting procedures such as: wound care, ECGs, phlebotomy, gynecological and genitourinary exams, incision and drainage, staple and suture removal, and others as indicated.
    • Creating patient-centered care plans that consider patient strengths and limitations, in collaboration with the interdisciplinary team.
    • Prescribing medicines as related to the patient care plan, within the primary care scope of practice, using internal and external consults (such as UpToDate, etc.) and using Janian internal practice standards as a guide.
    • Utilizing internal and external consults and supports to guide patient care, such as RubiconMD, clinical supervision, Epic Carelink, UpToDate, UCSF Warmline, Janian clinical guidance documents, and others.
    • Utilizing clinical harm reduction and a non-judgmental approach to clinical care.
    • Collaborating and coordinating with agency stakeholders in re: patient care, clinic workflows, supply ordering, and others.
      • Agency stakeholders include but are not limited to medical assistants, LPNs, RNs, social workers, case managers, psychiatrists, other primary care providers, medical directors, Janian administration, CUCS administration.
    • Communicating with the interdisciplinary team routinely and in a timely manner, based on internal practice standards.
    • Documenting patient visits as per agency policy and internal practice standards.
    • Developing and utilizing leadership skills to manage the primary care clinical practice at each assigned program site.
    • Maintaining current licensure and board certification.
    • Following all Janian and CUCS policies and procedures.
    • Maintaining good working relationships with coworkers and agency partners.
    • Other duties as needed and/or as assigned by the clinical supervisor, Medical Directors, or Director of Primary Care.
Administrative Responsibilities - 40% of position
  • Administrative responsibilities will fill 2 days of a 5-day work week.
  • Supervise Janian Primary Care staff.
    • In collaboration with the Director of Primary Care, guides the introduction and integration of primary care and nursing operations into new programs.
    • Supervises primary care providers, nurses, and other Janian staff in the primary care department as assigned by the Director of Primary Care.
    • Supervisor duties will include, but not limited to:
      • Ensure staff practice safely and effectively within their clinical scope.
        • Advise and coach staff around clinical care during routine supervision meetings.
      • Ensure staff work effectively in a multi-disciplinary team.
        • Ensure staff maintain good working relationships with Janian staff and agency partner staff.
        • Advise and coach staff on interpersonal and leadership skills during routine supervision meetings.
      • Ensure staff document their work per agency policy.
        • Advise and coach staff on agency requirements, internal practice standards, and time management skills during routine supervision meetings.
      • In collaboration with Janian leadership and contracted partners, ensure primary care operations are effectively integrated into program site operations.
        • Ensure effective and appropriate utilization of Janian primary care staff at each level (PCP, RN, LPN, MA);
        • Liaise with agency partners to optimize workflows and collaboration between Janian primary care staff and agency partner staff.
        • Collaborates with Janian administrators and the Director of Primary Care to ensure appropriate administration of this program.
  • Ensure appropriate, effective day-to-day primary care and Street Medicine Team (SMT) operations at Breaking Ground programs
    • In collaboration with other agency leadership, ensure primary care and SMT operations are effectively integrated into program site operations.
    • Develop and implement policies and procedures for safe, effective primary care practice that complies with all regulatory standards, supports best practices and fulfills contract obligations.
  • Attend, and participate in, and/or lead Breaking Ground agency meetings as directed by the Medical Director for Psychiatry - Breaking Ground.
  • Liaise between primary care, psychiatry, and social services leadership and staff at Breaking Ground programs.
  • Provide consultation to Breaking Ground staff about primary care and health related issues.
  • Maintain good working relationships with Breaking Ground and Janian partners, funders and other stakeholders.
  • Effectively participates in agency committees, such as Janian's DEIB committee, the CUCS Consumer Advisory Board committee, incident review committee, and others as assigned by Janian leadership.
  • Effectively participates in Janian and/or Breaking Ground workgroups, such as the Primary Care Billing Workgroup, Primary Care Subcommittee workgroups, Quality Improvement workgroups, and others as assigned by Janian leadership.
  • Assist in recruiting and hiring primary care staff including registered nurses, licensed practical nurses, physicians and nurse practitioners.
    • Screen resumes and contact potential candidates.
    • Conduct recruiting calls, phone screenings, interviews, and coordinate and/or host site visits as assigned by Janian leadership.
  • In collaboration with the Development Department and other agency leadership, help pursue and secure funding for primary care programs.
    • Assist in writing proposals for possible grants.
    • Participate in meetings with the development department, potential and current funders, and other agency stakeholders.
    • Create, run and present reports as assigned by agency leadership, in collaboration with agency stakeholders, to secure or maintain grant funding.
    • Collaborate with Janian Primary Care leadership to ensure appropriate administration of this operation.
Requirements
  • Board Certified and licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty.
  • At least three years of primary care clinical experience.
  • At least one year of relevant clinical experience working with patients experiencing homelessness.
  • Experience working with electronic medical records (experience with eClinicalWorks preferred).
  • Proficiency with Microsoft Excel, Word and Outlook.
  • Excellent organization, communication and collaboration skills.
  • Commitment to servant leadership principles and person-centered practices.
