The Sentara Heart Hospital located on the Campus of Sentara Norfolk General is hiring a Transplant Coordinator, Registered Nurse for the Heart Failure Program

This position is full time.

The outpatient clinic is open Monday - Friday between 8am and 4:30pm.The Transplant Coordinator, RN will have regularly scheduled call, and has weekend call about every 8th week.

Relocation assistance can be provided to qualified candidates.

What you will do:

  • Coordinates the clinical management of the transplant/VAD patient including the transplant/VAD evaluation, the transplant/VAD process and follow-up care.
  • Facilitates clinic flow, transplant evaluation, and patient care management.
  • Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
  • Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
  • Transplant Coordinator will manage a case load of patients, remain patient's point of contact through the transplant process, recovery and at home.
  • Troubleshoots heart equipment device.

Education/Certification/Licensure:

  • Registered Nurse, BSN Required.
  • RN should have 2+ years of acute care experience, good medical - cardiac knowledge, and understand chronic disease.
  • BLS required.
  • RN License Virginia or Compact/Multi-State Eligible
  • Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
