What You Will Do

As a Hereditary Hemorrhagic Telangiectasia (HHT) Advanced Practice Provider, you will be responsible to medically manage adult patients with HHT in collaboration with the IHTC physicians.

The Opportunity

  • Triage/management of patient issues in the following areas:  on-call, inpatient, and other outpatient setting.  Management of HHT related complications, including epistaxis, iron deficiency anemia, and screening for arteriovenous malformations.
  • Effective cooperation and communication with other HHT team members to ensure all patient needs are met, including phone triage.
  • Patient management includes but not limited to health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, surgical care planning, education/counseling, ongoing monitoring and follow-up.
  • Documentation in electronic medical record per policies of IHTC and St. Vincent Hospital.
  • Identify at-risk family members of affected patients and coordinate screening/testing.
  • Work with patients to successfully transition from the pediatric medical providers to adult providers at the appropriate age/developmental level.
  • Perform procedures, as delegated with competency check off by the physician.
  • Collect outcomes and data for applicable grant reporting.
  • Authorized to administer Basic Life Support in accordance with current CPR standards.

Requirements

  • Minimum Master’s degree
  • Minimum 3 years of experience working with patients with a chronic disease
  • Valid IN Nurse Practitioner or Physician Assistant (and, if applicable, PA-C) License
  • Valid driver’s license and insured automobile required

Our center has more than 50 clinical research projects involving bleeding disorders, sickle cell disease, thrombosis and more.
