Position: Street Outreach Coordinator

Reports To: Deputy Director

Salary Grade: $17 per hour

FLSA Hourly: Non-Exempt

The Street Outreach Coordinator will participate in daily street outreach to homeless clients in the community they serve by providing essential services necessary to reach out to unsheltered homeless people. In conjunction with community partners, the Coordinator will provide both outreach/engagement services by linking clients to longer term housing, health, mental health and other supportive services. The Coordinator will attend and facilitate community meetings and work alongside community partners to ensure implementation of Coordinated Entry System (CES) across a specific geographic region. The coordinator will network with local businesses and organizations to provide referrals and linkages to necessary services as well as provide transportation to clients accessing services. Furthermore, street outreach projects must comply with any assessment requirements detailed in the applicable written standards adopted by the Continuum of Care.

Requirements

Necessary Requirements of Work

Graduation from an accredited two-year college or university with an A.A or A.S. Degree in Sociology or related behavioral/social fields. A Bachelor degree in the area of social work is preferred. Additional knowledge of VI-SPDAT and SOAR is a plus but not required. Must have experience working with individuals from a variety of socio-economic backgrounds and considerable experience performing administrative functions; good interpersonal, organizational and decision-making skills; considerable experience working with the public. Applicant must have effective written and oral communication skills, knowledge of general office equipment and possess suitable computer skills for data entry. Any equivalent combination of education or experience to provide the following essential functions is required.

Essential Functions

  • Locate, identify, and build relationships with unsheltered homeless people
  • Conduct daily street outreach to develop a relationship that will assist clients with addressing barriers to obtaining services such as housing and health care needs
  • Make an initial assessment of needs and eligibility, provide crisis counseling, and address urgent physical needs
  • Conduct and complete all necessary paperwork to track client progress
  • Provide street case management services by bringing services to the homeless population
  • Provide access to urgent, non facility-based care
  • Provide access to outpatient treatment by licensed professionals of mental health conditions operating in community-based settings, including streets, parks, and other places where unsheltered people are living.
  • Provide transportation of unsheltered people to emergency shelters or to other eligible service facilities
  • Be prepared to implement a variety of interventions to secure permanent housing
  • Perform Habitability Inspections of units for clients assisted with ESG and/or CoC housing assistance
  • Assist with housing applications as well as supportive and subsidized housing paperwork
  • Assist the client with benefits establishment (GR, SSI, TANF, Veterans pension, etc.) by obtaining documentation such as ID, birth certificate, and social security cards
  • Use and develop community resources to broker and link clients to services
  • Ability to sit, stand, climb stairs, walk, drive, stoop, bend, reach, carry (15lbs), and lift (15lbs)
  • Maintain client files per program requirements
  • Performs related duties as required.
