Responsibilities The RN Navigator works closely with patients across all aspects of the continuum of care (primary care physicians, specialists, ancillary services, inpatient and outpatient facilities, wellness programs, etc.) to coordinate a care plan program. Under general administrative direction, they give patient education related to the patient's health maintenance, preventative care and planned treatment. As a single point of contact, the Transition Nurse will provide medical information, education and share knowledge regarding internal and external support services and resources. The transition nurse will monitor patient outcomes and participate in quality improvement activities. They will contribute to and collaborate with health care team members to positively impact patient outcomes and patient experiences. Norton Healthcare is committed to establishing health equity in every neighborhood, and Norton West Louisville Hospital is an example of that commitment put into action. Opening November of 2024, this state-of-the-art facility is being built at 850 South 28th Street and will offer comprehensive services, including a 24-hour emergency department, inpatient care, operating rooms and imaging technology. It will serve both adults and children. It's designed with the voice of the community in mind, and will change health care for generations to come.
