An experienced heath care professional; (for example Registered Nurse, Occupational Therapist, Social Worker et cetera) Certified Diabetes Educator with skills and knowledge who utilizes the nursing process to provide direct and indirect patient/participant diabetes education. Responsible for program planning, implementation, and evaluation. Educator and resource person to patients and staff. Works independently with little supervision.

Required qualifications:
- Graduation from an accredited healthcare professional program (RN, SW, PT, OT, etc.).

- Certified Diabetes Educator (CDE) - National Certification Board for Diabetes Educators within 1 year of hire
- 2 years of Diabetic self care management experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team
Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
