Responsibilities:

  • Track and report on key performance indicators (KPIs) for case management activities.
  • Collaborate with quality improvement initiatives, ensuring seamless patient transitions across care settings.
  • Stay current on the latest trends and best practices in case management.
  • Lead and mentor case management staff, promoting professional development and a positive work environment.

Qualifications:

  • Active Registered Nurse (RN) license in the state of Iowa or a multistate license allowing work in Iowa.
  • Minimum of two years of experience in Case Management, with a focus on utilization management, case management, discharge planning, or other cost/quality management programs.
  • Two to three years of previous management experience in a healthcare setting is preferred, with a minimum of two years in hospital-based nursing.
  • Master's degree in Nursing (MSN) or a Bachelor of Science in Nursing (BSN) with a strong background in case management is highly desirable.
  • Proven leadership skills with the ability to motivate, inspire, and guide a team.
  • Excellent communication, interpersonal, and collaboration skills.
  • Strong analytical and problem-solving skills.
  • Proficient in electronic health record (EHR) systems and case management software.
  • Passion for patient-centered care and commitment to continuous quality improvement.
