The Community Psychiatric Rehabilitation (CPR) Treatment Team Leader provides clinical guidance, supervision, and training for the assigned treatment team. The Team Leader ensures the completion of all required documentation, adhering to all accrediting bodies and agency standards and expectations.
This position provides effective leadership to staff by creating a positive work environment and ensures that all services are delivered in a strengths-based and person-centered manner.
**JOB DUTIES AND RESPONSIBILITIES**
* Hires, trains, and provides supervision and oversight to assigned Community Support Specialists (CSS) and Behavioral Health Clinician.
* Completes the annual assessment and wellness plan and all other required assessment and documentation as outlined by program standards and regulations, within the electronic medical record (EMR) for clients on assigned team.
* Ensures all services are delivered and documented in a timely and clinically appropriate manner in the electronic medical record per agency policy.
* Oversees staff performance/productivity
standards and provides coaching and problem-solving to resolve areas of concern.
*
Addresses client concerns and crisis situations while providing clinical guidance to treatment team members.
* Provides oversight regarding the direct services assigned to designated team (disease management, ITCD, etc). Direct services may include intensive problem-solving, case-management services, supported employment and crisis-intervention as needed.
* Assures that admission/continued stay criteria are met as outlined by program standards and regulations. Completes discharge summaries when appropriate.
* Actively participates in the quality improvement process by conducting chart reviews and monitoring of required documentation in the electronic medical record (EMR).
* Provides direct services when needed to the departments assigned clients.
* Provides consultation/education
for the community.
* Participates in and follows all agency and department policies/procedures regarding treatment and documentation practices, adherence to DMH and Medicaid standards, QA/QI procedures, and emergency procedures. Reviews the treatment and documentation practices of their assigned team.
**QUALIFICATIONS**
* Master's degree in psychology, social work, counseling or closely related human services field;
* or a Bachelors degree in a human services field (social work, psychology, nursing, education, criminal justice, recreational therapy, human development and family studies, counseling, child development, gerontology, sociology, human services, behavioral science and rehabilitation counseling) and meets the qualifications of a Community Support Specialist with an additional three years of experience providing community support services to individuals with mental illness or substance use disorders in accordance with the community support key service functions.
* Minimum of two years experience in the behavioral health field. Minimum of three years experience in the behavioral health field for those with a bachelors degree.
* Licensed Clinical Social Worker or Licensed Professional Counselor preferred, but not required.
