WestCoast Children's Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

The Intensive Care Coordinator (ICC) is responsible for engaging and assisting clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family's identified goals.

The ICC coordinates, monitors and contributes to the activities of all parties involved in the care and treatment of the child and family to support and ensure successful and enduring change consistent with the CFT Plan. ICCs also provide a diversity of care through conducting short term needs and strengths assessments and providing stabilization support services to children and TAY in a milieu environment. An ideal candidate demonstrates flexibility, initiative, creativity, inclusivity and knowledge of the unique challenges youth and young adults face in foster care. This position also provides services and participates in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)
How you'll make an impact:

  • Ensure the provision of Intensive Care Coordination (ICC) that is responsible for working as equal partners with family, natural supports, and formal system partners to facilitate a family-centered, strength-based, culturally sensitive, needs and strengths driven planning process.
  • Regularly schedule, organize and facilitate Child and Family Team (CFT) Meetings with youth/families and their community stakeholders. Intensive facilitation training will be provided.
  • Conducts short term strengths and needs assessments to empower youth in case planning.
  • Provide stabilization and comprehensive care to foster youth in a 24 hr drop-in center as youth await and prepare for transition to foster placements.
  • Completes all necessary medi-cal documentation written in a timely manner (within 2 days of services), consistent with all standards and regulations.
  • Represents our mission, philosophy, and upholds ethical and legal responsibilities as a mandated reporter
  • Participates in individual and group supervision, program meetings and trainings consistently and on time.
  • All other duties as assigned by supervisor or program director.
Qualifications:
  • Bachelor's degree in Psychology, Criminal Justice, Social Work, or a related field preferred.
  • Experience with foster care, social services, juvenile justice, mental health, or related community resources is highly desirable.
  • 2 years of direct experience with young adults (TAY) and/or families in a mental health context.
  • Desired skills: facilitation, needs assessment, crisis intervention, and de-escalation.
  • Knowledge of wraparound or team-based care services.
  • Passion for supporting young adults with complex trauma.
  • Eagerness to learn about crisis intervention, child welfare systems, mental health, and local resources.
  • Strong organizational, written, and communication skills (writing sample required).
  • Values teamwork, cultural humility, compassion, inclusivity, and social justice.
  • Flexible schedule, with availability for evenings and occasional weekends.
  • Clean California DMV record; local travel and car required.
Competencies (skills, abilities, and knowledge):
  • Experience working with transitional age youth (TAY) and families in the child welfare system.
  • Strong case management skills, including assessment, crisis intervention, supervision, and high-quality documentation.
  • Cultural humility with an understanding of race, class, and ethnicity issues.
  • Efficient time management and organizational skills with timely documentation.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Competence in supporting clients with complex trauma and attachment issues.
  • Professional conduct.
Position details and compensation: This is a full time, hourly position. Salary: $65,000 per year ($71,500 per year for bilingual Spanish). Includes competitive benefits package, 403(b) plan, and performance-based compensation.
