Description
1. Provides services to persons with severe and persistent mental illness who qualify for services under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
2. Provides services as specified on the weekly schedule. Team members have the ability to provide a variety of functions and must demonstrate a willingness to be flexible in their duties.
3. Provides support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan.
4. Provides assistance in developing a) social skills, b) natural support systems, c) adaptation skills, d) skills related to developmental issues, e) daily living skills, f) work readiness activities, and g) leisure time activities.
5. Provides general health care monitoring, supportive counseling, recovery/resiliency support, symptom management, and budgeting skills.
6. Provides services in the individual's natural/community setting 75%-85% of the time.
7. Participates in daily organizational meetings to review service contacts within the past 24 hours and action plan for the current day.
8. Participates in agency MAP teams and staffings when indicated.
9. Maintains client confidentiality at all times.
Documentation and Quality Improvement:
10. Documents appropriate activities in case note format that justify the need for treatment.
11. Maintains case record documentation that is adequate, appropriate, complete, and timely.
12. Completes and submits service logs per agency policy.
13. Develops the Recovery Support Plan with input from the multi-disciplinary team. Assists in development of the Individual Service Plan and Crisis Support Plan.
Interpersonal and Leadership Skills:
14. Responds to clients needs in an appropriate and timely manner.
15. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
Support Duties:
16. Completes additional job duties assigned by supervisor.
17. Participates in interagency collaboration for the provision of client services when assigned.
Key Responsibilities
ESSENTIAL QUALIFICATIONS/SKILLS/EXPERIENCE:
Available 24/7 as assigned to respond to needs of individuals enrolled in the I-CORT program.
Requires DMH certification within 30 days of hire and maintained throughout duration of employment.
Requires excellent time management, organizational, and interpersonal skills.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within 30 days of employment.
Requires valid driver's license and auto liability insurance.
Requires ability to transport clients in private vehicle when necessary.
Requires completion of annual training in compliance with DMH standards.
PREFERRED QUALIFICATIONS/SKILLS/EXPERIENCE:
Previous experience in a mental health setting strongly preferred.
Previous experience working with an SMI population in a treatment setting strongly preferred.
Working knowledge of the DSM.

Bachelors
Minimum Requirements
Bachelor degree in behavioral science or related field. Eligible for Community Support Specialist certification through the Mississippi Department of Mental Health.
Licensure/Certification
Requires DMH certification within 30 days of hire and maintained throughout duration of employment.
