The Behavioral Health Home Organization Nurse Care Specialist (NCS) carries out medical oversight to specific clients enrolled with the BHHO team. Medical functions include basic psychiatric health assessments, treatment, monitoring of medications, education, and coordination of health care provided to clients in person. The BHHO NCS will also be responsible for assuming the lead role in developing and directing rehabilitation services to a designated caseload of clients.
Duties include and are not limited to:
+ Assessing the medical needs of clients and coordinating services for the clients based on these needs
+ Providing education around medical diagnoses and prescribed medications specific to clients
+ Being present for assessments related to higher levels of care such as PSS, Skilled Nursing, Assisted Living etc.
+ Making necessary referrals for high levels of care when applicable
+ Assisting with the development and implementation of treatment plans goals targeted towards addressing medical needs
+ Providing back up coverage for the BHH Nurse Care Manager when needed
+ Completing all notes for direct and indirect contact regarding clients on a daily basis
+ Setting up an Health Info Net patient list and following up on all notifications timely
+ Providing support and education to the case management team members regarding cases they need support with
+ Monitoring medical and psychiatric medication side effects and coordinating with providers to support client in decreasing negative effects
+ Performing other duties as assigned
Qualifications
Position Requirements
+ Proof of vehicle insurance at $50K/$100K and a valid Maine drivers license are required
+ Must be 18 years of age or older
+ Associate's Degree required. Bachelor's Degree is preferred.
+ Minimum of three years clinical nursing experience is required
+ Must hold a current license to practice in the State of Maine as a Registered Nurse (RN), Advance Practice Nurse (APRN), Licensed Practical Nurse (LPN) or Clinical Nurse Specialist (CNS)
+ Intermediate level computer experience
