Responsibilities:

The incumbent provides direct social work services, referrals to community services and counseling to patients and their families, and functions as a member of the interdisciplinary team.

  • Provide psycho social patient assessments.
  • Develop collaboration with team, patient, family, and physician. POC to address patient needs as related to diagnosis.
  • Provide appropriate intervention to include: Referrals to community agencies, assist patient with completion of forms, etc., provide counseling, advocacy role with outside resources, financial counseling, assist with appropriate living environment.
  • Document outcomes on Plan of Care and make necessary changes. Documents follow-up of identified problems.
  • Evaluate Plan of Care, make changes or adapt as patient and family needs change.
  • Collaborate and educate team members to ensure appropriate psycho social intervention/staff support to process referrals, i.e., CPS/APS/Ombudsman, etc. Demonstrates coordination of care by appropriate communication with other disciplines and documents communication.
  • Work as a team member to meet the department goals and objectives.
  • Meets discipline specific productivity standard.
  • Demonstrates competency in the care of patients/families during field observation by clinical supervisor.
  • Physician orders are written according to policy.
  • Reviews and documents clinical progress toward goals.
  • Documents appropriate management of visit utilization and authorization status as measured by a designated evaluation tool.
  • Completes and submits re-certification and/or discharge documentation following established guidelines.
  • Develops and presents one in-service a year.

Qualifications:

Minimum Requirements:

Education and Experience:

  • A minimum of two (2) years in a health care setting which provides for the following knowledge, skills and abilities.

Licensure:

  • LCSW license
  • Current BLS through the American Heart Association.
  • Current California Drivers License and access to an operating motor vehicle.

Ability to:

Demonstrate knowledge and skills relevant to the principles of growth and development over the life span of the patient. Interpret knowledge needed to identify each patients requirements relative to his or her age specific needs. Perform psychosocial assessments and formulate appropriate plans. Assess and report incidences of suspected child abuse, dependent adult/elder abuse and domestic violence. Communicate psychosocial needs of the patient and family to the interdisciplinary team. Develop and maintain a professional working relationship with peers and medical staff members. Develop and maintain a professional relationship with community health, welfare and social agencies. Develop and present one inservice per year. Participate in 1 QM/UR study/committee per year. Maintain patients medical records. Maintain confidentiality.

Knowledge of:

Principles and practices of clinical social work in the healthcare setting. Individual and group consultation process. Principles of grief and loss
counseling. Crisis intervention techniques. Patients rights, ethical considerations and cultural differences in dealing with thehomebound patient. Community health, welfare and social agencies. Chemical dependency and mental disorders.
