Provides patient care and case coordination to persons with HIV infection including pre and post test counseling, education of patient, family/significant others, staff and community. Assesses need for medical, social, psychological interventions and facilitates appropriate referrals; established working relationships between community based service providers and hospitals to assure continuity of care.
