The ideal candidate will be self-directed, capable of working both independently and collaboratively to implement care based on established care management principles, focused on assessing needs, identifying health disparities, and overcoming barriers to care.

Candidates for this role should have 3-5 years of relevant clinical experience across various healthcare settings, including inpatient and outpatient care. Utilization management experience is preferred.
