To be successful in this role, Case Managers will need excellent communication and relationship-building skills, knowledge of community resources, and comfort with making home visits and traveling locally throughout the workday. This particular position is part of our PACT team in Yakima. PACT (Program for Assertive Community Treatment) is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The PACT model is characterized by a team approach, shared caseload, time-unlimited services, flexible service delivery, and 24/7 crisis availability.
