Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We hire people that are skilled and passionate to meet our mission through outcome-based strategies which support personal and community transformation.

SUMMARY: HDSC provides both medical and counseling support for individuals in a 60 bed medically monitored subacute detoxification program. This position requires working directly with individuals in withdrawal management, completing a thorough assessment, documenting progress notes, and providing discharge planning. The primary focus is assessing for the next appropriate level of care and connecting patients with services to meet their treatment and recovery needs. The successful candidate will have experience with motivational interviewing, case management and a knowledge of the impact of homelessness and addictions on adults and families. This position has access to mental health billing and service records.

Location: Hooper Clinic

On-Call-On Call employees must work at least once in every 6 month period to remain active on payroll.

RESPONSIBILITIES:

* Perform and review assessments based on ASAM - PPC 2R criteria and provide diagnostic impressions based on DSM V criteria. * Develop a needs and service plan with a focus on discharge planning. * Interview and interact with clients to increase motivation. * Work with Hooper Clinical Team, Nursing, and Subacute Technicians to accomplish treatment, wellness and recovery goals. * Refer individuals to appropriate treatment programs and community support for follow-up care. * Facilitate patient access for direct contact to service agencies. * Participate in direct treatment of individuals and attend clinical staff meeting. * Coordinate transportation for individuals as needed. * Participate in quality assurance and quality improvement activities as directed. * Work directly with supervisor for ongoing program and individual professional development. * Maintain appropriate and timely client records and clinical documentation. * Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

DESCRIPTION OF OTHER DUTIES:

* Conduct treatment groups as needed. * Develop resource information to be used by the team. * Perform other duties as assigned.

ABILITIES:

* Demonstrated skills in counseling techniques with individuals and groups. * Demonstrated knowledge of community and social service agencies. * Experienced in Motivational Interviewing with ability to promote Change Talk. * Knowledge of substance use disorders, co-occurring disorders and treatment modalities. * Knowledge of crisis intervention methods and de-escalation techniques. * Knowledge of stages of change and strengths perspectives. * Sufficient manual dexterity and physical ability to perform assigned tasks. * Ability to effectively communicate with clients, co-workers, medical personnel, police, merchants, and the public at large. * Ability to manage time and meet deadlines. * Ability to maintain accurate records and necessary documentation. * Ability to provide leadership.

QUALIFICATIONS:

* High School Diploma or GED required. * The following requirements for CADC: *

SUDS Counselor R requires CADC-R * SUDS Counselor I requires CADC-I * SUDS Counselor II requires CADC-II

* * Case management or referral specialist experience in addition to A&D Counseling preferred. * Ability to work with multi-disciplinary team. * Must meet CCC privileging requirements as required by FTCA. * Must possess at least a high school diploma or equivalent. * CPR certification required prior to start date. * Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol. * Must pass a pre-employment drug screen, TB test, and criminal background check. This includes clearance by the DHS Background Check Unit. * The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act. * Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift up to 25 pounds. * Must adhere to agency's non-discrimination policies. * Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
