JOB DESCRIPTION:The Care Coordinator is responsible for coordination of activities related to a specific patient population and the administration of direct patient care for a specific unit and shift. The Coordinator functions as a leader of multidisciplinary teams, providing and coordinating care to patients and families. Concurrent utilization and service review, consultation, patient and family education, promotion of continuity of care, preceptorship and committee work are required at this level of practice.EDUCATION:Current RN licensure or compact licensure recognized by the State of Vermont is required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.EXPERIENCE:Previous experience working in an Operating as a registered nurse is required.
