Description
SUMMARY
The Intake/Discharge and Rapid Resolution Specialist (IDRRS) is primarily responsible for engaging Veteran clients seeking assistance with housing and services. The IDRRS will also provide more intensive case management services for case load of approximately 25 individuals. The Specialist is also responsible for direct outreach and pre-screening to Veterans who may be appropriate for SSVF. This includes the provision of rapid resolution, a diversion and rapid exit strategy to see if a Veteran has other options.
PRIMARY DUTIES
- Engage clients to understand services provided by the SSVP program and the process of obtaining services through Baltimore City Homeless Services Department and other Veterans services.
- Screen and triage individuals to determine eligibility for Rapid Resolution Program and SSVF.
- If appropriate, provide rapid resolution, a diversion and rapid exit strategy to see if a Veteran has other options.
- Complete initial client Intake and Assessment using the HMIS "Client Track".
- Complete the B-DAT on all clients seeking housing placement assistance.
- Develop an individualized housing and service plan with a goal of obtaining permanent housing.
- Assist clients with obtaining mainstream benefits and legal documents.
- Work closely with the Program Director in assigning clients to Case Managers.
- Up-date Resource Directory, forms and other documents used to provide direct client assistance.
- Monitor and evaluate client progress as outlined in the Individualized Service Plan.
- Provide information and referrals to other providers to assist with meeting the needs of the client.
- Attend monthly all staff meeting and other department meetings as assigned.
- Process Temporary Financial Assistance in accordance with required limitations.
- Utilize HMIS for data collection and comply with HMIS and community standards for data entry. This may require entry into more than one HMIS.
- Participate in active networking with other homeless service providers, as well as other community resources.
- Attend monthly staff meeting and other department meetings as assigned.
- Supports a welcoming and positive environment for volunteers and partner agencies.
- Upholds the vision, mission and values of St. Vincent de Paul.
- Assists with and attends special events held by the Center and St. Vincent de Paul.
- Other duties as assigned within the scope of the position.
QUALIFICATIONS
- Bachelor's degree in Social Work or another related field.
- 2-3 years of experience in working directly with persons who are materially poor and/or homeless in an urban setting.
- Ability to relate in a courteous, comfortable and non-judgmental manner with persons from all ethnic, socioeconomic and religious backgrounds.
- Understanding of and respect for confidentially.
- Strong computer skills
- Strong organizational and time management skills.
- Good communication and interpersonal skills.
- Desire and ability to work as a member of a team.
- Demonstrate strong knowledge of the resources in the community available for the Veteran population, especially services and programs offered by the VA
- Possess strong knowledge of homelessness, severe and persistent mental illness, and substance abuse
- Provide positive customer service to all including difficult and challenging populations
- Must be sensitive to and respect cultural diversity amongst clients, staff, and volunteers and able to work with diverse racial, ethnic, and economic groups
