SUMMARY:Primary responsibility for this position is to serve as the first point of contact for individuals seeking help through the family resource center and being able to connect them with internal programs and external partners. This position will be knowledgeable about programs offered through the family resource center, eligibility requirements, and services offered throughout the community. ESSENTIAL DUTIES:Provides a friendly and compassionate first point of contact to visitors and building partners.Provides needs-related screening and processing of paperwork (Intake Forms, ASSM, SNAP) of program applicants and clients, as required within assigned programs. Schedules or assists in scheduling necessary client appointments to access services. Takes inquiries and referrals from the Catholic Charities website, email, and phone and refers to Case Manager, co-locating partners, and community resources.Develops relationships with co-locating partners and community service providers to understand the current resources that are readily available to North Nashville individuals and families. Collaborates with Case Manager and co-locating partners to achieve program referral and enrollment goals.Ensures the effective maintenance, tracking, and reporting of files and data as required by program contracts.Performs administrative tasks such as answering phones and email inquiries, data entry, copying, filing, and ordering of office supplies.Assists Case Manager and staff with office calendar, tracking and scheduling events and available meeting rooms for co-locating partners and community organizations. Coordinate all referrals for program participants and scheduling of appointments as needed.Provide clients with the necessary social service assistance, and referrals to appropriate internal resources and have knowledge of other community resources not offered by Catholic Charities. Maintains complete and timely case notes according to Agency guidelines, using the database provided by the agency and within Agency time frame.Assist other team members on an as-needed basis.Actively participates in supervision sessions, periodic team meetings, and training.Performs other duties as assigned. RequirementsQUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
