Job Summary:

Works closely with patients to navigate the developed care plan while providing the highest patient centered care. Acts as a liaison between hospital administration and patients when concerns are presented. Responds and addresses outpatient and inpatient care concerns and provides assistance, consultation, education and guidance to identify the patient's needs to provide solutions and assistance as necessary. Has substantial understanding of techniques, processes, procedures to perform the job and seeks resolution in creative and effective ways. Runs program(s) with moderate budget/impact.

Job Requirements:

Education and Work Experience:
  • High School Education/GED or equivalent: Preferred
  • Associate's/Technical Degree or equivalent combination of education/related experience: Preferred
  • Five years' relevant experience: Preferred
Licenses/Certifications:
  • Valid Driver's License (DL) and must be at least 21 yrs of age or older: Preferred
  • Driver's License (DL): Required
Essential Functions:
  • Assists patients in navigation of the unique needs of the patient care plan. Responds to and addresses patient care concerns in both the inpatient and outpatient settings.
  • Provides high level of patient service in accordance with the values and expectations of the organization. Coordinates with the patient, administration, physicians, internal departments and all other internal and external teams to achieve optimal success of patient care plan.
  • Manages patient inquiries to ensure appropriate resources are provided, monitors for timely follow-up and resolution, if applicable. Works to resolve conflict by talking directly with involved parties to bring about resolution.
  • Documents all escalations in accordance with department policies and system requirements.
  • Follows appropriate chain of command in achieving resolution. Adheres to all company policies, procedures, industry standards and agency regulations and rules.
  • Performs other job-related duties as assigned.
