Responsibilities:

  • Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life
  • Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population
  • Assist the Quality Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies, prepare assigned information for input in to the MDS computer program

Qualifications & Experience Requirements for Social Worker:

  • Previous Experience as a Social Worker preferred 
  • Must have a Valid Social Worker License
  • Must have Bachelor's degree from accredited school of Social Work or related field
