Position Overview:
Seeking an experienced Case Manager to join the team and manage a client case load, specific to clients at high risk of psychiatric hospitalization or interventions. Job Description:
+ Communication and relationship building with client and all collateral partners connected with each client (schools, funding partners, therapists, agencies, clinicians, supervisor, foster homes, etc.).
+ Setting limits for youth, development of interventions, setting goals toward individual successes in conjunction with clients guardians, therapist and other providers.
+ Initiate and monitor external recommendation processes and treatment planning for clients in case load.
+ Ensure compliance with policies and procedures, including all contract sources.
+ Interact in a timely and effective reporting as directed for computer reports, tracking, paper reports and the like in accordance with agency protocols.
+ Participate in on-call services as needed.
+ Represent Agency on task forces and community boards regarding FBS interests.
+ Possess and exercise strong interpersonal skills throughout all levels of Agency.
+ Attend meetings throughout Agency and with collateral partners.
+ Keep abreast of trends, significant events in the field of Human Services and Case Management.
+ Analyze situations, identify problems, identify and evaluate alternative courses of action.
+ Plan and organize work effectively and ensure its completion.
+ Use discretion and individual judgment with agency under the guidance and direction of supervisor.
+ Maintain highest level of confidentiality.
+ Responsibly, accurately, and completely document all work and share as appropriate within FBS and with collateral partners.
+ Other duties as assigned related to these primary responsibilities.
Qualifications:
Bachelor's Degree Required
LSW or LSWA or Licensed Eligible
Must have valid driver's license and verification of personal auto insurance
Schedule: Monday - Friday 9:00am-5:00pm (may work weekends if needed to accomodate client)
