Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Behavioral Health team The team is seeking a dynamic Medical Assisted Treatment Supervisor to assist, communicate and implement innovative integrated primary care behavioral health (PCBH) and Patient Centered Medical Home (PCMH) programming under the supervision of the Senior Clinical Manager and Director of Behavioral Health.

Main Duties & Responsibilities
  • Care Coordination
    • Administrative: 8 hours a week dedicated to clinical administrative follow up and 4 hours a week dedicated to leadership oversight.
    • Work with the BH Manager and/or Director of Behavioral Health to plan, organize and direct activities of MAT program across different sites. Will assist with development and adherence to the program goals and objectives, reinforces program procedures, and implements programming to meet clinical and agency need.
    • Ensure the highest quality of care by working with an interdisciplinary team to: (1) maintain program policies and procedures; (2) identify and eliminate barriers to care; and (3) participate in quality assurance activities, including chart reviews, program evaluation, staff development trainings, and outcome analysis.
    • Supervise volunteers and interns working within the clinic setting and provides clinical supervision and guidance to direct staff. Develop, participates, and maintains an "on-call" procedure in order to address BH emergencies.
    • Clinical: 28 hours a week dedicated to direct client contact.
    • Provide individualized treatment and/or group mental health support for Erie patients who are experiencing behavioral/MA health issues.
    • Screen children, youth and adult primary care patients, as requested by the PCP, and develop a plan of action to address presenting needs and assist with the implementation of the PCP treatment plans.
    • Offer and coordinate internal/external mental health referrals and community resources for primary care patients. Follow-up and monitor completion and outcomes of referral for at-risk patients.
    • Assist PCPs to understand, develop and implement short-term mental health interventions to improve patient treatment adherence and achievement of health goals.
  • Data Management
    • Create and maintain protocols and workflows to ensure clear and consistent guidelines for the program.
    • Assist with the design, development, and maintenance of the patient tracking tools and the registry for MAT patients. This database will have up to date information such as date/duration of last medication, dates of last UDS and medical visit, date of last substance use and EHR record number and date of birth.
    • Maintain patient enrollment and progress toward improved mental health


Qualifications

Education
  • Master's degree in a Behavioral Health discipline (e.g. Social Work, Psychology, Mental Health Counseling, etc.) with a valid Illinois State Licensure as appropriate to discipline is required.
  • LCSW or LCPC required
  • CADC a plus
  • Local travel required, valid Illinois driver's license and automobile insurance also required
Skills and Knowledge
  • Minimum of 5 years of experience working within a medical/clinic setting highly preferred or at least one year working as a Behavioral Health Consultant on an FQHC primary care team is required.
  • Minimum of 3 years of experience working within substance use patients highly preferred
  • Program management experience and proven administrative leadership highly preferred
  • Demonstrated experience in crisis intervention, brief counseling, and psychoeducation
  • Demonstrated cultural competency working with diverse communities, in a low income, minority environment
  • Must be able to demonstrate strong communication, organization, and written skills (included in Leadership Skills)
  • Demonstrates knowledge about the PCBH + PCMH model
  • Demonstrates knowledge and application of behavior change theories, techniques for behavioral modification, brief interventions, motivational interviewing, CBT and other BH evidenced based treatment approaches
  • Community resources oriented
