The Care Coordinator will serve patients, most of whom have chronic disease, behavioral disorders, and medical conditions. The Care Coordinator functions at the center of the patients' whole-person care, preventing fragmented care and integrating all aspects of patient care and treatment. The Care Coordinator monitors and tracks progress and ensures that changes are affected when needed. The Care Coordinator is responsible for assuring that all of the services and providers of the patients' care and care team are working together.
