The role youll contribute: The Care Coordinator serves as an advocate and resource for the patient, their family/caregiver and their health care team to coordinate care needs and community resources to improve health outcomes and decrease the overall cost of patient care. Facilitates a "shared goal model" within and across settings to achieve coordinated high-quality care that is patient[1]and caregiver-centered. Builds effective relationships in the community and across the medical continuum of care to strengthen care and social services coordination. This individual works with outpatient practices to standardize screening around social and structural determinants of health (SSDH), coordinate referrals to community social service partners with a focus on reducing and eliminating health disparities for Kentucky's most vulnerable citizens.

The value youll bring to the team:

  • Facilitates patient access to different community support and financial assistance programs (i.e. AdventHealth's financial assistance program, scheduling transportation to appointments, picking up medications and following up with care providers about their condition) enabling them to access the care they need.
  • Addresses and works to resolve patient concerns or barriers to treatment plan adherence.
  • Assesses and identifies strategies to improve the health literacy of the patient and, as appropriate, family and other caregivers. Assists patients and caregivers with social determinants of health needs including access to exercise and healthy food.
  • Advocates for and provides evidence to support upstream investments (including grants) to further social determinants experimentation.
  • Ensures effective collaboration and communication between the patient and members of the healthcare team to support care coordination activities and goal attainment (i.e. appointment details, health reminders, access to medication, decreased inappropriate use of the emergency room for non-emergent care).
  • Serves as a point of contact, advocate, and informational resource for patient, family, care team, payers, and community resources and facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed.
  • Promotes and participates in organization and community activities focused on the health of a patient and their community, including outreach, quality improvement and patient panel management.
  • Contributes and participates in Population Health Management activities (i.e. care gap closures) of the organization, infusing the lens of health equity into the work.

Qualifications

The expertise and experiences youll need to succeed:

  • 3-5 years' experience in clinical or community health settings in Care Coordination, Case Management or Home Health experience
  • Experience with navigation of local medical and social support systems
  • Bachelor's Degree in Social Work, Communication, or a related field
