Care Coordinator

ResponsibilitiesThe Medical Assistant Care Coordinator functions as an integral member of the multidisciplinary team of care providers. The Care Coordinator is responsible for tracking abnormal results and coordination of follow-up care based upon providers recommendation. The Case Manager is responsible for coordinating the interdisciplinary team of providers to best serve the patient. This process includes assessment, education and continuity of care for the patient and family. The Medical Assistant works closely with the Medical Director of Cancer Center, Nurse Manager of the Cancer Center, Nurse Navigators, RN Clinical Site Coordinator and appropriate specialists to coordinate patient services and outreach components of the program. This includes monitoring patients through screening programs and oversees that the patients get the appropriate follow-up care and treatment that is required.
