Full Time Patient Liaison - 1241

A growing HCBS (Home and Community-Based Services) company is seeking a proactive and personable Patient Liaison to join their team. This role is focused on creating and maintaining strong relationships with external partners and referral sources to drive patient referrals and enhance the company's service reach.

Responsibilities Include:
  • Relationship Building: Develop and maintain relationships with healthcare providers, community organizations, and other potential referral sources.
  • Referral Management: Actively seek out and secure new patient referrals by engaging with existing and new contacts.
  • Communication: Serve as the main point of contact for referral sources, ensuring clear and consistent communication.
  • Networking: Attend events, meetings, and conferences to promote the company's services and establish new connections.
  • Reporting: Track referral activities and provide regular updates to management on the status of relationships and referral numbers.
  • Collaboration: Work closely with internal teams to ensure seamless onboarding and care for referred patients.
Ideal Qualifications:
  • Experience: Previous experience in a similar role, preferably in healthcare or HCBS services.
  • Interpersonal Skills: Strong ability to build and maintain relationships with a variety of stakeholders.
  • Communication Skills: Excellent verbal and written communication skills.
  • Organizational Skills: Highly organized with the ability to manage multiple relationships and tasks simultaneously.
  • Results-Driven: Motivated to achieve referral targets and contribute to the company's growth.
