Physical Therapist (PT) As a Physical Therapist at Pinnacle Home Care, you’ll play a vital role in delivering high-quality care to patients in the comfort of their homes. You’ll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards. Provide education and support to patients and families on therapy programs. Document all visits and progress notes on the same day of service. Qualifications Graduation from an accredited Physical Therapy program. Active PT license in Florida. At least one year of PT experience (home health experience preferred). Proficiency with electronic medical records is a plus.
