They work closely with clients, families, and medical professionals to ensure each assigned client receives Always Best Care's industry leading care services. The Client Care Manager is responsible for creating, updating, and reviewing client care plans, as well as maintaining communication with clients and updating the company's client reporting/communication portal. They also serve as a liaison between clients and medical and health professionals, such as primary care physicians, emergency care providers, physical and occupational therapists, home health providers, and hospice services. Additionally, the Client Care Manager ensures client care consistency, including the quality of care, team dynamics, and overall client care satisfaction. They handle incident and grievance reporting, client care scheduling, and reporting on client care needs and opportunities. The Care Manager is supervised by the Director of Operations, with the President serving as a secondary supervisor. The right candidate will meet the following criteria: * Will be an LPN, RN, or have a Bachelor's Degree in Social Work, Healthcare Administration, or Psychology, or have the right combination of ability and experience. * Possess excellent tech skills, fluent in the use of smart phones, tablets, computers, and office equipment. * Knowledgeable and fluent in the use of Office Programs such as Microsoft Word, Excel, and Outlook. * Willing and able to learn all company systems including webbased programs. * Have excellent communication skills both in writing and in spoken word. Possess the ability to discuss issues of a sensitive nature over the phone and in person. * Must possess a polished appearance and professional manner. * Must be very organized, systematic, and systems oriented. * Must be a self starter, have a proactive nature, and willing and able to make decisions and have tough discussions with kindess, grace, and firmness. Expected pay is posted but is negotiable to the right candidate.
