- Position Type: 24 hrs per week with Sat-Sun and some weekdays
- Pay Range: $55- $60
- Department: Director of Nursing

The MDS RN will be responsible for conducting comprehensive assessments, collecting data, and collaborating with the interdisciplinary team to develop individualized care plans. This position also coordinates with other healthcare professionals to ensure accurate documentation and coding according to federal and state regulations. Attention to detail and strong analytical skills are essential for this position, as you will be responsible for accurately assessing the patient's functional status and medical needs.

**Responsibilities**:

- Conduct comprehensive assessments and evaluations to determine the patient's eligibility for services
- Collect and analyze patient data to identify areas of improvement in care
- Collaborate with the interdisciplinary team to develop individualized care plans
- Ensure accurate and timely completion of MDS assessments
- Coordinate with healthcare professionals to ensure accurate documentation and coding
- Stay updated with federal and state regulations related to MDS assessments
- Provide education and training to staff regarding MDS documentation and coding

**Requirements**:

- Valid RN license in CA
- Minimum of 1 years of experience as an RN in a long-term care setting
- Knowledge and understanding of the MDS assessment process and regulations
- Proficiency in electronic medical records (EMR) and MDS software
- High attention to detail and strong analytical skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced, team-oriented environment
- Strong organizational and time management skills
