Our organization is seeking qualified **Registered Nurse (RN) - Hospice Case Manager **to work with our geriatric population in their own homes or within Independent Living Facilities. Responsibilities include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families, and the administrative nursing team to ensure a continuum of care.
Additionally, you will have the opportunity for **salary increase** upon completion of your CHPN (Certified Hospice & Palliative Nurse) at no charge to the employee. We support and encourage professional development within our team.
- Willing to travel to multiple locations
- Valid RN (Registered Nurse) License in good standing with the State of Michigan
