**General responsibilities**:
- Provide insight, support and advocacy to clients assisting them to access, utilize, and follow through with treatment, social service, medical, vocational, housing, and associated community resources that promote recovery.
- Minimum of 20 hours spent on site monitoring and interacting with the clients.
- Will serve as the liaison between landlord (FCRHRA) and the program.
- Scheduling and conducting intake screenings for individuals seeking services at Moltenbrey.
- Manage and monitor the waitlist.
- Development of individual service goals with the creation of an IAP.
- Meet with clients to review treatment plan, set goals, facilitate referrals, and evaluate progress on IAP.
- Organize and facilitate a minimum of 2 psycho-educational, Health & Wellness, and Life Skills groups.
- Complete enrollment & discharge summaries in virtual gateway.
- Facilitate admissions of new clients.
- Complete clear and objective admission and discharge summaries on clients.
- Create and maintain accurate and up to date MAR (medication Administration Record).
- Provide education about HIV/AIDS, tobacco use, MAT and access to health services.
- Monitor participant progress and provide supports and problem-solving strategies to facilitate recovery and sobriety.
- Attend community meetings as they present themselves, such as OTF, and provider meetings.
- Monitor and update client insurance in virtual gateway.
- Train, guide and support other case manager in effective and positive case management.
- Report client progress and needs to Clinical Supervisor.
- On-call responsibilities is required.
**Additional responsibilities**:
- Report admissions and discharges to Intake Coordinator.
- Respond quickly and effectively to emergency situations.
- Collaboration with community providers.
- Write clear and objective log notes, case management notes, service notes and quick notes.
**Schedule**:
- Monday
- Friday: 8:30am-4:30pm
- One shift will be 10:30am-6:30pm
**Pay Schedule**:
- $24-25 per hour
All staff are responsible to respond quickly and effectively to emergency situations, facilitate and monitor substance use screening with clients, perform CPR/First Aid when needed, scanning, and uploading client documents and uphold client confidentiality and following applicable HIPAA and 42 CFR Part 2 guidelines.
In addition, all staff are required to attend staff meetings, individual supervision manager. Trainings to meet program, agency, and BSAS guidelines throughout the year are mandatory for continued employment. Missing a training that ServiceNet has paid for will result in staff having to pay for the makeup training.
This description covers the essential functions of the position. Incumbent is expected to perform other similar and related duties as assigned. Management has the right to change or add to the job responsibilities at any time.
